While some scoliosis cases are caused by congenital structural abnormalities or neurological or muscular diseases, the vast majority\u2014around 85%\u2014are of unknown cause.<\/p>\n<p style=\"font-size: 19px; line-height: 29px;\">The fact is, the bones in our body do not move unless our muscles move them. And our muscles are controlled by our nervous system. <strong>So when our vertebrae move out of alignment in any way, they are being moved by our muscles, which are being controlled by our nervous system.<\/strong><\/p>\n<p style=\"font-size: 19px; line-height: 29px;\">Many cases of idiopathic scoliosis are caused by chronically tight muscles pulling the spine out of alignment. <strong>If you have idiopathic scoliosis, you can touch your back and waist and feel how tight your muscles are.<\/strong><\/p>\n<p style=\"font-size: 19px; line-height: 29px;\">If your nervous system is sending messages to your muscles to stay tight, no amount of passive lengthening (such as static stretching or massage) or forced re-alignment (such as bracing or chiropractic) will change these messages.<\/p>\n<p style=\"font-size: 19px; line-height: 29px;\">In this post, we&#8217;ll talk about the patterns of muscular contraction that are common in idiopathic scoliosis, how our natural motor learning process leads us to develop these patterns, and how <strong><a href=\"https:\/\/somaticmovementcenter.com\/pandiculation-what-is-pandiculation\/\" target=\"_blank\" rel=\"noopener noreferrer\">pandiculation<\/a><\/strong> retrains the nervous system to release chronic, involuntary muscular contraction.<\/p>\n<p style=\"font-size: 19px; line-height: 29px;\"><i><b>&#8220;This is the best program I have encountered.<\/b> I am a military veteran, with 2 tours to Afghanistan and have scoliosis which became much worse due to all the heavy lifting and carry overseas. If the spine curves in both directions, it is described as an S-curve. If the degree of curvature is eleven degrees or more, it will be diagnosed as scoliosis.<\/p>\n<\/div><div class=\"fusion-clearfix\"><\/div><\/div><\/div><div class=\"fusion-layout-column fusion_builder_column fusion-builder-column-2 fusion_builder_column_1_2 1_2 fusion-one-half fusion-column-last\" style=\"--awb-bg-size:cover;width:50%;width:calc(50% - ( ( 4% ) * 0.5 ) );\"><div class=\"fusion-column-wrapper fusion-column-has-shadow fusion-flex-column-wrapper-legacy\"><div class=\"fusion-text fusion-text-3\"><p><img decoding=\"async\" class=\"alignleft size-full wp-image-8983\" src=\"https:\/\/somaticmovementcenter.com\/wp-content\/uploads\/2017\/10\/ScoliosisCurves.jpg\" alt=\"Scoliosis C curve S curve\" width=\"1366\" height=\"768\" srcset=\"https:\/\/somaticmovementcenter.com\/wp-content\/uploads\/2017\/10\/ScoliosisCurves-300x169.jpg 300w, https:\/\/somaticmovementcenter.com\/wp-content\/uploads\/2017\/10\/ScoliosisCurves-768x432.jpg 768w, https:\/\/somaticmovementcenter.com\/wp-content\/uploads\/2017\/10\/ScoliosisCurves-1024x576.jpg 1024w, https:\/\/somaticmovementcenter.com\/wp-content\/uploads\/2017\/10\/ScoliosisCurves.jpg 1366w\" sizes=\"(max-width: 1366px) 100vw, 1366px\" \/><\/p>\n<p style=\"font-size: 17px; line-height: 27px;\"><em>Left: C-curve; Right: S-curve<\/em><\/p>\n<\/div><div class=\"fusion-clearfix\"><\/div><\/div><\/div><div class=\"fusion-layout-column fusion_builder_column fusion-builder-column-3 fusion_builder_column_1_1 1_1 fusion-one-full fusion-column-first fusion-column-last\" style=\"--awb-bg-size:cover;\"><div class=\"fusion-column-wrapper fusion-column-has-shadow fusion-flex-column-wrapper-legacy\"><div class=\"fusion-text fusion-text-4\"><h2><strong>Types of scoliosis<\/strong><\/h2>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">Approximately 85% of scoliosis cases are classified as <strong>idiopathic<\/strong>,<sup>1<\/sup> which means that the cause of the spinal curvature is unknown.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">In cases of <strong>congenital scoliosis<\/strong>, the spinal curvature is a structural abnormality that is present at birth.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">In cases of <strong>neuromuscular scoliosis<\/strong>, the spinal curvature is caused by a neurological or muscular disease, such as cerebral palsy, spinal cord trauma, muscular dystrophy, spinal muscular atrophy, spina bifida, neurofibromatosis, or Marfan syndrome.<\/p>\n<\/div><div class=\"fusion-clearfix\"><\/div><\/div><\/div><div class=\"fusion-layout-column fusion_builder_column fusion-builder-column-4 fusion_builder_column_1_1 1_1 fusion-one-full fusion-column-first fusion-column-last\" style=\"--awb-bg-size:cover;\"><div class=\"fusion-column-wrapper fusion-column-has-shadow fusion-flex-column-wrapper-legacy\"><div class=\"fusion-text fusion-text-5\"><h2><strong>Side effects of scoliosis<\/strong><\/h2>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">Roughly two-thirds of adults with scoliotic curves between 20 and 55 degrees experience back pain.<sup>2<\/sup> Many people with scoliosis develop pain in other parts of their bodies due to their postural misalignment, which puts uneven stress on the hips, knees, neck, and shoulders. Arthritis, disc and nerve compression in the spine, and difficulty breathing are also common.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">Many people with untreated scoliosis, as well as some who have been surgically treated, develop <strong>spondylosis<\/strong>.<sup>2<\/sup> Spondylosis is an arthritic condition of the spine in which joints become inflamed, cartilage thins, and bone spurs develop. Disc degeneration or spinal curvature can lead to spinal vertebrae pressing on nerves, resulting in severe pain and requiring surgery.<\/p>\n<\/div><div class=\"fusion-clearfix\"><\/div><\/div><\/div><div class=\"fusion-layout-column fusion_builder_column fusion-builder-column-5 fusion_builder_column_1_1 1_1 fusion-one-full fusion-column-first fusion-column-last\" style=\"--awb-bg-size:cover;\"><div class=\"fusion-column-wrapper fusion-column-has-shadow fusion-flex-column-wrapper-legacy\"><div class=\"fusion-text fusion-text-6\"><h2><strong>Bracing and surgery\u2014are they effective?<\/strong><\/h2>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">Typically, bracing is recommended for curves greater than 25 degrees. Bracing attempts to slow or halt curve progression by forcibly aligning the spine. However, studies show mixed results when it comes to the efficacy of bracing,<sup>3<\/sup> and some experts believe that the practice of bracing is outdated and ineffective. Patients who wear braces can also experience negative side effects such as pain, restricted breathing, and weakening or stiffening of their muscles due to lack of movement.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">When curves progress to 45-50 degrees or more, spinal fusion surgery is considered. About 38,000 people undergo spinal fusion surgery each year in the United States.<sup>4<\/sup> In spinal fusion surgery, metal rods, hooks, wires, and screws are attached to the spine in order to force it into a straight position. Then doctors attach pieces of bone, which grow together and create the actual fusion of the spine. Patients who undergo this type of surgery lose 20 to 60 percent of their spinal flexibility.<sup>5<\/sup> A great deal of strain is put on the unfused parts of the spine, leading to a high rate of disc degeneration and osteoarthritis. Research shows that 75% of patients experience degeneration in their <strong><a href=\"https:\/\/somaticmovementcenter.com\/si-joint-pain-dysfunction\/\" target=\"_blank\" rel=\"noopener noreferrer\">sacroiliac joints<\/a><\/strong> after spinal fusion surgery.<sup>6<\/sup> And sadly, more than 40% of spinal fusion patients experience no reduction in their pain levels.<sup>5<\/sup><\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">Rates of complications in spinal fusion surgeries vary, but are quite high across the board. Some research has shown that more than half of the surgeries are unsuccessful, meaning that the vertebrae do not actually fuse. Even though the vertebrae are held in place by hardware, patterns of contraction in the back muscles cause micromovements in the spine, preventing continuous growth of the bone. Muscular contraction can be so strong that metal rods inserted along the spine actually break, causing a great deal of pain and requiring repeat surgery. Given the high risk of complications and the lack of evidence supporting spinal fusion as an effective treatment, many doctors and researchers now agree that the surgery can be used to slow or halt progression of the curvature, but little else.<\/p>\n<\/div><div class=\"fusion-clearfix\"><\/div><\/div><\/div><div class=\"fusion-layout-column fusion_builder_column fusion-builder-column-6 fusion_builder_column_1_1 1_1 fusion-one-full fusion-column-first fusion-column-last\" style=\"--awb-bg-size:cover;\"><div class=\"fusion-column-wrapper fusion-column-has-shadow fusion-flex-column-wrapper-legacy\"><div class=\"fusion-text fusion-text-7\"><h2><strong>Rates of idiopathic scoliosis rise dramatically with age<\/strong><\/h2>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">A 2014 review published in American Family Physician found that approximately 85% of scoliosis cases are classified as idiopathic, or of unknown cause.<sup>1<\/sup> So, 85% of the people who get diagnosed with scoliosis get no explanation as to what has caused it.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">The same review found that between 2% and 4% of teenagers have scoliosis. According to a retrospective study done at Johns Hopkins University, the rate of scoliosis increases to more than 8% in adults over the age of 40.<sup>7<\/sup> And a 2005 study of 75 healthy adults over the age of 60, with no previous diagnosis of scoliosis or spinal surgery, found the rate of scoliosis to be 68%.<sup>8<\/sup><\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><strong>This increasing prevalence with age is a strong indicator that muscular contraction from repetitive activities, injury, and stress\u2014the effects of which increase with age\u2014play a role in developing the condition.<\/strong><\/p>\n<\/div><div class=\"fusion-clearfix\"><\/div><\/div><\/div><div class=\"fusion-layout-column fusion_builder_column fusion-builder-column-7 fusion_builder_column_1_1 1_1 fusion-one-full fusion-column-first fusion-column-last\" style=\"--awb-bg-size:cover;\"><div class=\"fusion-column-wrapper fusion-column-has-shadow fusion-flex-column-wrapper-legacy\"><div class=\"fusion-text fusion-text-8\"><h2 class=\"fusion-responsive-typography-calculated\" style=\"--fontsize: 22; line-height: 1.45; --minfontsize: 22;\" data-fontsize=\"22\" data-lineheight=\"31.9px\"><strong>The complex muscular patterns involved in idiopathic scoliosis<\/strong><\/h2>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">Our spine can flex (bend) in all directions: forward, backward, and to each side. When we bend in any direction, we can rotate our spine to either side as well. Since we have 24 articulating (moving) vertebrae in our cervical, thoracic, and lumbar spine, and many different muscles that control the movement of our vertebrae, we can develop varied, unique patterns of spinal flexion and rotation.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><strong>Scoliosis is never as simple as a single sideways bend or curve in the spine.<\/strong> We always compensate or balance ourselves out by developing other muscular patterns, like rotating to one side, arching our back, or rounding forward.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><strong>Let&#8217;s talk about some of the most important muscles involved in scoliosis:<\/strong><\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">The biggest, strongest muscles in our core that flex our spine laterally are our <strong>internal and external obliques<\/strong>. Our obliques also rotate our spine, so any chronic tension in our obliques will likely create both a lateral curve and some degree of rotation.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">The <strong>erector spinae group<\/strong> of muscles travels from the base of the skull and cervical vertebrae all the way down to the pelvis, attaching to each vertebrae and rib. This group of muscles both laterally flexes the spine and extends it, meaning that it arches the back. So, chronic tension in these muscles can create both scoliosis and hyperlordosis, or an exaggerated arching of the lower back.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">The <strong>intertransversarii<\/strong> are small, short muscles that connect each individual vertebrae to the vertebrae above and below it in the cervical and lumbar portions of the spine. These little muscles laterally flex the cervical and lumbar spine. Since they are the deepest muscles in the neck and lower back, they are nearly impossible to touch or sense internally.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">We think of the <strong>quadratus lumborum<\/strong> (QL) as a lower back muscle, but technically, it&#8217;s our deepest abdominal muscle. The QL attaches our lowest rib to the top of our pelvis, and connects to our first through fourth lumbar vertebrae. This strong muscle laterally flexes our spine to either side, laterally tilts our pelvis (hikes our hips up one at a time), and helps to extend our spine. So, chronic tension in the QL not only contributes to lumbar scoliosis, but also to <strong><a href=\"https:\/\/somaticmovementcenter.com\/uneven-leg-length-the-myth-of-uneven-leg-length\/\" target=\"_blank\" rel=\"noopener noreferrer\">functional leg length discrepancy<\/a><\/strong> and hyperlordosis.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">The <strong>latissimus dorsi<\/strong> is the broadest muscle of the back, spanning from the lumbar and lower half of the thoracic spine all the way to the upper arm just below the shoulder joint. The latissumus dorsi participates in many actions: extending, adducting, and medially rotating the shoulder, laterally flexing the spine, extending the spine, and even tilting the pelvis forward or to the side.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">The <strong>transversospinalis group<\/strong> of muscles are small muscles in between each vertebrae, similar to the intertransversarii. But this group of muscles rotates and extends the spine, contributing to both scoliosis and hyperlordosis.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><strong>The most important thing to take away from this discussion is the fact that the muscular patterns involved in scoliosis can be very complex.<\/strong> All of the muscles in the core of the body, including some we haven&#8217;t mentioned, will be involved in the lateral flexion, rotation, and extension or forward flexion of the spine, as well as in the compensatory patterns we develop in order to balance ourselves out.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">The result of so many multi-functional muscles being involved is that idiopathic scoliosis typically involves lateral flexion to one or both sides, rotation to one or both sides, and either or both extension and forward flexion. These turns and twists can occur at various parts of the spine, creating patterns of spinal curvature as unique as we are.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><strong>In the next section, we&#8217;ll talk about how and why we develop these complex patterns of muscular contraction.<\/strong><\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><b>And <a href=\"https:\/\/www.youtube.com\/watch?v=wT7oeUUHLOU&amp;t=2s\" target=\"_blank\" rel=\"noopener noreferrer\">watch this video<\/a> to learn how chronic muscle contraction pulls the spine into a curve.<\/b><\/p>\n<\/div><div class=\"fusion-clearfix\"><\/div><\/div><\/div><div class=\"fusion-layout-column fusion_builder_column fusion-builder-column-8 fusion_builder_column_1_1 1_1 fusion-one-full fusion-column-first fusion-column-last\" style=\"--awb-bg-size:cover;\"><div class=\"fusion-column-wrapper fusion-column-has-shadow fusion-flex-column-wrapper-legacy\"><div class=\"fusion-text fusion-text-9\"><h2 class=\"fusion-responsive-typography-calculated\" style=\"--fontsize: 22; line-height: 1.45; --minfontsize: 22;\" data-fontsize=\"22\" data-lineheight=\"31.9px\"><strong>Why we develop the involuntary muscle contraction that causes idiopathic scoliosis<\/strong><\/h2>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">As I mentioned at the beginning of the post, our bones don&#8217;t move unless our muscles make them move. And our muscles are controlled by our nervous system. So if any of the bones in our body are moving out of alignment, it&#8217;s because our nervous system is sending the message to our muscles to move our bones in that way.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><strong>Why would our nervous system send the message to do something that could cause harm to our body? And why do some people develop scoliosis while others don&#8217;t?<\/strong><\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">The movement and level of contraction of our muscles is controlled by our nervous system. The way that our muscles move, and how much we keep them contracted, is actually <em>learned<\/em> over time by our nervous system.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">Our nervous system learns certain ways of using our muscles based on how we choose to stand and move each and every day. <strong>Our nervous system notices the postures and movements that we tend to repeat, and it gradually makes these postures and movements automatic so that we don\u2019t have to consciously think about them.<\/strong><\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">This learning process\u2014that of developing what we refer to as <strong><a href=\"https:\/\/somaticmovementcenter.com\/muscle-memory-and-sensory-motor-amnesia\/\" target=\"_blank\" rel=\"noopener noreferrer\">muscle memory<\/a><\/strong>\u2014allows us to go through the activities in our daily lives easily and efficiently. Unfortunately, if we tend to repeat unnatural postures or movements, our nervous system will learn those too. <strong>Our automatic neuromuscular learning process doesn\u2019t discern what is good or bad for us\u2014it just notices what we tend to repeat, and makes it automatic.<\/strong><\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">So if you carry your bag on the same side every day, hold your child on the same hip, sit at your desk in the same way, or adapt your posture to an injury\u2014the muscles that flex your spine to the side can easily become chronically tight, leading to idiopathic scoliosis.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">As your nervous system gradually learns to keep your muscles tight, <strong><a href=\"https:\/\/somaticmovementcenter.com\/gamma-loop\/\" target=\"_blank\" rel=\"noopener noreferrer\">gamma loop<\/a><\/strong> activity adapts. This feedback loop in your nervous system regulates the level of tension in your muscles. As your brain keeps sending the message to contract your muscles, gamma loop activity adapts and starts keeping your muscles tight all the time. Meanwhile, your proprioception (your internal sense of your posture and movement) adapts so that you\u2019re not aware of the increased level of tension in your muscles or your altered posture.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">Many people develop involuntary muscle contraction that pulls their spine out of alignment in some way; both postural kyphosis (rounded back) and hyperlordosis (arched lower back) are very common. Lateral flexion is also very common, but many people&#8217;s lateral curvature remains less than 11 degrees throughout their life, so they don&#8217;t get diagnosed with scoliosis.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><i>&#8220;After suffering several injuries to my body, including a back injury, my S-curved spine became more severe and painful. I tried various exercises, physical therapy and an online posture course for scoliosis. Short of having back surgery, recommended by an orthopedic doc, nothing worked. <\/i><\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><i>I hit the jackpot with the Somatic Movement Center. <b>After the first month of doing exercises in the Scoliosis Course, my back and sciatica pain are basically gone.<\/b> The sciatica pain only flares up occasionally if I lift a heavy object or sit on a hard surface. The movement technique of pandiculation allows you to do all of these things by sending accurate feedback to your nervous system about the level of tension in your muscles.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><strong>Pandiculation releases subconsciously held muscular contraction and brings muscles back into voluntary control.<\/strong> Thomas Hanna incorporated the technique of pandiculation into his system of neuromuscular education called Clinical Somatic Education. Attempting to improve a functional spinal curve by using manual force, like a brace or hardware, is nonsensical and most often ineffective, as it does not change the messages that your nervous system is sending to your muscles to stay tight.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">Likewise, manual therapy like chiropractic and massage does not change the messages that your brain is sending to your muscles. <strong>Changing these messages requires retraining your nervous system using active, conscious movement: namely, pandiculation.<\/strong><\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\">Idiopathic scoliosis patients who use Clinical Somatics exercises to release the chronic muscular contraction that is causing their curvature typically experience reduction or elimination of their pain as well as gradual straightening of their spine. When you stand up, you\u2019ll feel unbalanced, but that\u2019s part of the learning and adjustment process that your nervous system needs to go through. Is one side tighter or looser than the other?<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><strong>You can then go back and forth from side to side, learning from your more coordinated side.<\/strong> If a movement feels easy or \u201cright\u201d on one side, try to replicate that feeling and way of moving on your other side.<\/p>\n<p style=\"font-size: 19px !important; line-height: 29px !important;\"><strong>Above all, be patient with yourself.<\/strong> Releasing the complex patterns of muscular tension that cause idiopathic scoliosis is like peeling an onion. You&#8217;ll discover many layers\u2014many patterns of tension\u2014along the way. It takes time for the nervous system and the tissues of the body to adjust to new ways of standing and moving, so don&#8217;t try to rush the process.
